How do I get copies of my medical records?
To obtain copies of your medical records, you will need to contact the Health Information Management department at 817-599-1180 or stop by the office located on the lower level of the hospital. Only the patient, Medical Power of Attorney or person given written permission (proper documentation must be provided) is allowed to obtain medical records. A valid picture ID is required for anyone requesting records.
